2008-2009 Kindergarten Registration

For convenience, Kindergarten registration and new first grade student registration for the 2008-2009 school year will be conducted during the month of March. Registration will take place at Hillendale Elementary, Chadds Ford Elementary, Pocopson Elementary, and Unionville Elementary Schools between 9:00 AM and 3:00 PM.

To enter Kindergarten, children must be five (5) years old before September 1, 2008. To enter first grade, children must be six (6) before September 1, 2008. To complete your child’s registration, please bring:

1. Your child’s birth certificate.

2. Your child’s immunization record, with the following required immunizations:

• Diphtheria and Tetanus – 4 or more properly spaced doses with at least one dose given on or after the fourth birthday.
• Polio - 3 or more properly spaced doses.
• Measles, preferably given with mumps and rubella – 2 properly spaced doses (first dose given on or after first birthday); at least one dose of mumps and rubella is required.
• Hepatitis B – 3 properly spaced doses.
• Chicken Pox ( Varicella) - at least one dose on or after first birthday or immunity from history of disease.

3. Physical and Dental Forms - Students entering school in Pennsylvania are required to have a physical and dental examination.  Pennsylvania state physical and dental exam forms are included in the registration packet and must be completed, signed by the physician/dentist and returned to the school prior to entry.  The physical and dental exam must have occurred within one year of the date of school entry.

4. Proof of residency, such as a tax bill (school or township), house purchase agreement, rental or lease agreement, etc.
