The Unionville-Chadds Ford school District (UCFSD) in partnership with the Chester County Health Department (CCHD) will be hosting a combination seasonal flu and H1N1 vaccine clinic. The clinic will occur on Tuesday, January 19th 2010 in the Unionville High School cafeteria. The clinic will be held from 2:00 - 6:00pm. Both seasonal flu vaccine and H1N1 vaccine will be available to students, staff and community members:

 

1. Seasonal flu vaccine will be free to students 18 years old and younger. Adults will be charged $20 payable in cash or by check. Reimbursement forms will be available.

 

2. H1N1 vaccine is free to all recipients

 

This clinic will also provide the second dose of H1N1 vaccine to district students 9 years of age and younger who received their first dose at the UCFSD clinic on November 23rd 2009. A signed parent consent form is required for the second dose.

 

Please review the flyer and vaccine information forms, and complete the applicable consent forms linked below. Completed consent forms are required for all persons receiving either vaccine.

Greetings Parents, Guardians and Students,

 

The following information contains the details for administering the H1N1 nasal spray live vaccine, manufactured by MedImmune, to our healthy middle and high school students. Unionville High School students will be able to receive the vaccine on Tuesday, December 15th and Patton Middle School students on Wednesday, December 16th. This will be accomplished primarily during the school day each day. As a reminder, the injectable vaccine was offered to all K-12 high risk students at our clinic on November 23rd and WILL NOT be offered to our secondary students during these two days.

 

Process to be followed:

 

  1. Please link to the Consent form, download and complete - parent signature required for all students. (if needed, hard copies available in the nurse’s office)
  2. Consent form due date to high school homeroom teacher / middle school Prime Time teacher is no later than Tuesday, December 8th.
  3. Please read the Vaccination Information Sheet (VIS) for the live vaccine.
  4. Unionville High School vaccination administration plan:
    1. Unionville High School Nurse’s office – before, during and after school times
    2. Before school 6:45am – 7:30am, between classes, during lunch, during study hall, 2:30pm - 3:30pm.
  5. Patton Middle School vaccination administration plan:
    1. Patton Middle School Nurse’s office – exclusively during the school day.
    2. Students receiving vaccination will be called down by team per schedule established in advance.

November 23 UCFSD H1N1 Vaccination Clinic

 

Due to receiving another supply of vaccine, we are now opening the clinic up to the following UCFSD students:

 

-ALL K-12 High Risk Students
-Kindergarten through and including 5th grade Students

 

Clinic Date/Time: Monday, November 23, 2009, 3pm - 8pm

Location: Unionville High School Cafeteria (Enter via the Gymnasium Entrance)

 

UCFSD will be hosting an H1N1 vaccine clinic on Monday, November 23rd, 2009. Clinic hours are 3:00PM – 8:00PM. The location of the clinic is the Unionville High School cafeteria. Due to additional vaccine received, we now have vaccine to accommodate our K-12 high risk students and all K-5 elementary students. No registration is necessary. Students in elementary and middle school must be accompanied by a parent / guardian. High school students may be unaccompanied. ALL students need a Consent Form with parent / guardian signature.

 

The injectable vaccine, manufactured by Novartis will be used for our high risk students and it has been received in multi-dose vials – single dose drawn individually for each injection. The nasal spray vaccine, manufactured by MedImmune will be used for our healthy K-5 students and is a single dose delivery system.

 

Please, if possible, download the Consent Form, complete and bring to the clinic on Monday. Please also read the appropriate Vaccine Information Statement (VIS) form as it applies to the type of vaccine to be received.

Greetings Parents, Guardians, and Students,


Please do not confuse this event with the county vaccine clinics for residents that are occurring this weekend in Delaware County and early next week in Chester County.

 

The following information concerns our upcoming UCFSD H1N1 vaccination clinic.

 

Details as follows:

  • Monday November 23rd, 2009
  • 3:00pm – 7:00pm
  • Unionville High School Cafeteria (enter via gym entrance)
  • Students eligible to receive the injectable vaccine are:
    • Previously registered K-12 high risk students
    • Kindergarten students
    • Pre-First and First grade students

Important – only those categories above will be provided vaccine.

 

Our initial allotment of 800 doses is the basis for this decision.

 

Elementary and middle school students must be accompanied by a parent or guardian. High school students may be unaccompanied.

 

Consent form with parent / guardian signature is required for all students receiving the vaccine. If possible, please download and complete Consent Form prior to arrival. Parents are also asked to read the Vaccine Information Statement (VIS).

 

The injection will be administered in the upper arm. Please dress accordingly for ease of vaccinating.

 

Students nine years and younger will require a second vaccination. CDC recommends this second vaccination four weeks after the first injection. Vaccine may, or may not be available through the school district for this second injection.

 

Everyone is highly encouraged to continue practicing appropriate hygiene techniques, exercise regularly, eat nutritiously and obtain proper rest. Most importantly, if you have a fever of 100 degrees or greater and other symptoms, stay home and do not return to work or school until symptom free for 24 hours. Contact your physician as necessary.

Greetings Parents and Guardians,

 

The following important information pertains to the UCFSD plan to provide the H1N1 vaccine to our student population: (please note that information is subject to change due to reasons beyond the school district’s control)
  • The UCFSD has received an initial shipment of 800 doses of the injection type H1N1 vaccine
  • The injection vaccine is appropriate for much of the general population to include individuals with certain pre-determined medical conditions that are deemed high risk. (high risk individuals cannot take the nasal spray flu vaccine)
  • The UCFSD vaccine plan will offer, on a voluntary basis, this initial allotment to our high risk students K-12.
  • The balance of the 800 doses will be offered to our Kindergarten students first and will move in ascending grade order from first to twelfth grade.
  • PLEASE NOTE – PEOPLE WHO SHOULD NOT BE VACCINATED FOR THE FLU, NASAL OR INJECTION, PER CDC GUIDELINES ARE THE FOLLOWING:
    • Severe allergy to chicken eggs (those with egg allergy can use other flu medications within 24 hours of developing flu symptoms – contact personal provider.
    • Severe reaction previously to flu vaccine.
    • Developed Guillain-Barre syndrome within 6 weeks of receiving a flu vaccination.
    • Children less than 6 months old.
    • Currently sick with a fever – wait until well to get vaccinated.
  • To efficiently implement our plan we request any family who has a child, or children in the categories below to complete the brief survey and register (CLICK HERE TO REGISTER). Please register by November 17. This will allow us to determine what grade(s) we can include in the initial clinic date along with the high risk students. The tentative clinic date / location is Monday 23 November 2009 beginning 3:00PM in the high school cafeteria.A consent form is provided for your convenience. Please complete it prior to your arrival at the clinic. Additionally, if your child falls into a category below, please do not hesitate to contact your personal provider as needed.
    • Chronic heart disease
    • Chronic lung disease – asthma, reactive airway disease
    • Diabetes
    • Kidney failure
    • Illness that weakens the immune system
    • Take medications that weaken the immune system
    • Children or adolescents receiving aspirin
    • Pregnant women
    • Students who are household contacts of infants younger than 6 months of age
    • Students who are household contacts of a person who is immunocompromised.
    • Allergic to any nasal spray components
    • Any other chronic disease
  • Upon gathering data submitted we will then communicate confirmation of our clinic plan details. Again, our current plan is to administer the vaccine beginning 3:00pm on November 23, 2009 using the high school cafeteria as our clinic site.
